Ok so apparently I threw out the package already and the trash was emptied. Soooo I did a little digging online and apparently they no longer make that style. I have to go down my parents house this week; I have a lot of leftover stock (installation type gear) there from when I had my car shop open, so I'll see if there are any more leftover.
Since I can't seem to find them anywhere… I'll talk to my friend and see what it would take to make them for us. He manages a machine shop, and has access to a WaterJet and other equipment like that. I'll find out what it would cost to make them.
In the mean time, if you don't mind doing a little custom work yourself… you can buy these StreetWire terminals:

Then instead of installing a fuse install a copper bar across them and that will provide you with the 2 extra posts to use.
Once I get the install complete I'll do a write up with the parts I used and so on. I'm pretty OCD, so most people won't have the need to to install their equipment this way. But after working in the car audio business and having a ridiculous amount of people bring me their cars to "fix" the installation some other clown installed, I just do everything right the first time… plus there's less likelihood of your vehicle burning to the ground if its installed correctly
