I have. Maybe weld up something that I can bolt on. I just wasn't sure if there was an easier solution and if so where I can get them. I appreciate the help. Thank you. I'll probably end up building some.
It actually does not look bad. I was concerned it would be sunk in to far and the d- rings would have an issue hitting the frame hole. But it looks good. Thank you for picture. I think that is what I'm going to go with. Seems like a great option.

True. What size tap did you use?I drilled out and threaded it. Unless you've got alien fingers, it'd be tough to get a nut up under there.

I used a 3/8"x4.5" grade 8 bolt, washer, and lock washer to attach the pucks. I went long in case I wanted to add an additional puck in the future. I originally got a 1/4" bolt, but decided it could be too thin.
No they sent me a while new one with a slightly different bend bending away from the diff
No. it was a bit off center. I had to adjust it. It's center now.
