Lots to unpack with all of these. Lots of blame to go around it seems. Air traffic controller was doing two people’s jobs. Helicopter should have had two crew chiefs in the back seats for a training flight. The pilot of the Airplane probably should have gone around for another approach when ATC changed his runway on final approach. There was a near miss the day before yet they were still flying a packed schedule and ATC supervisor OKs one of the controllers to leave early.

I have seen many theories but the Control Tower watched it happen. 30 seconds before impact the tower asks the chopper pilot if he sees the plane. Then doesn't say much until about 3 seconds before impact and then, much to late, everyone gets to panic mode. I did see that the chopper was supposed to be flying from between 0-200' altitude and the impact happened at 350' so depending on what's true there is fault to go around. The traffic tower should have been commanding the flight path of the chopper no matter what the chopper pilots altitude instructions were. Very sad!
